We provide services for patients by appointment only. Please call our main number at (719) 633-8773, to schedule, cancel, or reschedule your appointment.
There is a $75.00 no-show fee if your appointment is not cancelled or rescheduled at least 24 hours in advance, unless there is an emergency or weather related issue that prevents a 24-hour notice. The no-show fee is required to be paid in full before another appointment can be scheduled.
There is a $250.00 no-show fee if your surgery or procedure at the hospital or surgery center is not cancelled or rescheduled at least 5 business days in advance. The no-show fee is required to be paid in full before another procedure or surgery can be scheduled.
We are contracted with most major insurance companies (please see our list). If you do not see your insurance on the list, please contact our office, as the list may have changed.
*TriCare requires a referral from your PCM at the Military Treatment Facility for pregnancy care. We cannot make an appointment for you without a current referral.
We require a copy of your insurance card(s) and a photo ID at every office visit, to verify the information to be what we currently have on file for you, since your last visit.
If you have Medicare coverage, we need a copy of your red, white and blue Social Security card and any Medicare supplement ID card, you have. If you have TriCare Standard, we need a copy of your military ID card. It must not be expired. If you have TriCare Prime, we also need a copy of your Prime card, in addition to your military ID card.
If you do not have verification of insurance coverage, we expect payment in full at the time of service, or we can reschedule your appointment to a time that you will have verification of insurance.
Forms of payment are: cash, personal checks, money orders for the exact amount, VISA and MasterCard. Post-dated checks are not legal and therefore, will not be accepted.

**This is the only facility where we deliver babies, due to the Neonatal Intensive Care Unit (NICU) that is located there. So, if your new-born baby needs that type of care upon delivery, mother and baby will not need to be transferred from another hospital.
