Insurance / Financial

We are contracted with most major insurance companies (please see our list). If you do not see your insurance on the list, please contact our office, as the list may have changed. *Tricare requires a referral from your PCM at the Military Treatment Facility for care. We cannot make an appointment for you without a current referral.

Prescription Refill Requests

Please contact your pharmacy first, and they will contact us with the request. If you need a new prescription or it has been over a year since you have seen us, an office visit will be required for any and all prescription requests.
